What do I need to provide to receive academic accommodations?

We require documentation confirming a diagnosis of disability and what accommodations are appropriate based on the impacts of your diagnosis on your learning. 

Some examples of documentation are an IEP (Individualized Education Plan), Psychoeducational Assessment, Audiologist report, or medical note.
